Documents for enrolment in the first grade:
- Enrolment application;
- Opinion of the inter-sectoral committee for the provision of additional educational, health and social support (given the current situation and recent extraordinary circumstances, a confirmation that the parents have submitted a request for the ISC opinion is sufficient);
- A doctor's certificate of the child's health status (an ophthalmologist's report with a recommendation on which script the child should use – Braille or not);
- One photograph.
Documents for enrolment in secondary school:
- a medical certificate confirming that the health conditions for enrolment in secondary school are met (an ophthalmologist's report with a recommendation on which script the child should use – Braille or not);
- the original certificate of the passed final exam;
- a secondary-school enrolment application (PU form);
- the original leaving certificate;
- one photograph;
- opinion of the inter-sectoral committee for the provision of additional educational, health and social support (given the current situation and recent extraordinary circumstances, a confirmation that the parents have submitted a request for the ISC opinion is sufficient);
- proof that the parents have begun the procedure for arranging payment of accommodation and meals in the school's Boarding House (if the student is interested in accommodation in the Boarding House);
- Medical-school students bring a confirmation that they are enrolled in the first grade of the Medical School (if the student is interested in accommodation in the Boarding House).
